At CanPharm Benefits we understand that privacy is an important issue for our clients and plan participants. The following information is designed to help you understand what information we gather, and how we handle the information once we gather it.
Information collection and use
CanPharm Benefits may receive personal information about you, including your name, address, e-mail address, personal identification number, social security number, home phone number, birth date, hire date, gender, termination date, etc. This information is used to enroll you in the CanPharm benefit plan and for claim administration, including payment status. As described below, we do our best to ensure that the personal information you provide to us is kept confidential. CanPharm Benefits will not share this information with others in ways different from what is disclosed in this statement. In addition, we will never sell or rent any of your information to others.
Security
All users are required to sign in to gain access to CanPharm Online.
Additional security measures protect your information once it reaches us. Only authorized CanPharm Benefits personnel have access to your information through integrated security.
Medical information
CanPharm Benefits will be communicating with you [in the following manner, including the fact that a receipt will be emailed]. Please be aware that unsecured emails could potentially be intercepted in transmission and read by somebody other than you. By agreeing to our process, you understand that you’re accepting that risk of someone other than you accessing the information in these emails.
CanPharm Benefits has taken the responsibility for adopting reasonable safeguards in implementing each individual’s request, however CanPharm Benefits is not responsible for disclosure of Personal Health Information while in transmission to the individual based on the individual’s access request to receive the PHI in an unsecure manner. This includes breach notification obligations and liability for disclosures that occur in transit. Further, CanPharm Benefits is not responsible for safeguarding the information once delivered to the individual.
CanPharm Benefits will safeguard all health care information we have in accordance with state and federal law, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We will not use or disclose health care information we have except as permitted by law.
Notification of changes
If we decide to change our privacy policy, we will post those changes on our website. If at any point we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will notify users by way of an email where an email is provided, or by regular mail. Users will have a choice as to whether or not we use their information in this different manner. The above privacy statement is effective as of March 1, 2008. CanPharm Benefits reserves the right to change this privacy statement from time to time. This privacy policy is not intended to and does not imply any contractual or other legal rights in respect to CanPharm Benefits or any other party.
